I’ve been a plex pass member for a relatively long time now, and have also built up quite the library. I’ve enjoyed the continued development as well.
Years ago, organization of TV Shows was simply "/tvshowname/tvepisode S##E##.
When did the requirement change to * TV Shows/ShowName/Season XX/ShowName – sXXeYY – Optional_Info.ext listed here: https://support.plex.tv/articles/200220687-naming-series-season-based-tv-shows/.
Do I need to go back and reorganize ALL my content?

I don’t know when the change happen but yes, reorganize IF you plan on rebuilding your plex server.
If you don’t and it still works then I say no, don’t do anything with the shows you currently have. Maybe use the “new” format going forward with new shows.
But don’t try to mix the old format with the new format on a series.
Again, this advise is if you have no issues with plex and your shows, and you don’t plan on rebuilding anytime soon.
It’s been that way since at least January of 2013. That’s when I built my Plex server, and that is how all my media has been organized ever since. Because it was the naming convention I had read in the documentation at that time, that we should use.
As long as the tv show in question is working as expected, probably not.
But I definitely recommend you to apply the proper folder organization for newly added content.
There is software which can take the “suck” out of this task.
