According to Intel, the E5-1660 doesn’t have QSV.
Did you pass the Nvidia into the container?
It looks like you did because it’s trying to talk to it but can’t.
Are the drivers up to date ?

Correct, docker container.
No Nvidia is being passed to the container (no card) – I did some further testing.
I downgraded to 1.22.0.4163-d8c4875dd and 1.21.4.4079-1b7748a7b – no difference in Firefox Web Player.
Tested on a few Apple devices, and Plex Windows Client – and plays the episodes fine.
Seem isolated to… Web Player (Tested in Firefox + Edge)
Thanks for digging deeper on that … CORS blocking sounds like the problem
FireFox and other browsers stepped up their security.
Give this a shot. It’s worked for others.

I let things play out - updated server to latest today (docker image) – and tested in firefox without addon, and seems to all be working again as before.
I guess / assume gremlins or elves worked / broke stuff behind to scenes.
All good now!
