Why Is HIMYM Episode Being Ignored?

I drop weekly episodes of American TV shows onto my QNAP NAS as they are obtained.  PMS running on the QNAP generally makes an excellent job at identifying and cataloguing the files.

 

But for some strange reason this weeks episode of "How I Met Your Mother"  (S09E06) is being totally ignored by the scanner. 

 

Why would this be, and how do I get the file to be scanned - at present I can't even see the file from within Plex/Web only in the QNAP File Manager?

2 things, make sure your naming convention is correct, and second verify your permissions on the file.
