Plex does not consider an „extras“ folder when looking for local extras. You might want to review your documentation;)
There’s a feature suggestion to add support for a more generic handling of local extras using a plain extras folder
Please show the complete folder path where the movie and this subfolder is stored.
At which folder level is the library pointed at?
Which Scanner and Agent type is selected in the properties of the library?
FIrst of all I would like to say that my folder structure hasn’t changed (in 10 years or even longer).
The library captures a list of individually defined letters like A,B,C,D…
Under each letter there’ s a movie whose name starts with the same letter as the parent folder.
If an only if I have some extras I have always placed them under a subfolder called Extras so:
So e.g.
/S/Stray dog (Nora inu) [1949]/Stray.Dog.1949.1080p.mkv
/S/Stray dog (Nora inu) [1949]/Extras/Wonderful To Create-Stray Dog.avi
The latter above is for some reason imported as a new movie called Extras in Plex.
While Plex is indeed supposed to ignore subfolders and files named “extra*”, this might have changed slightly with the introduction of the new “Plex Movie” scanner and agent. Now, your additional “index letter” subfolder level might throw a spanner in the works.

While thinkering I triggered a library scan and now I can see this:
I guess these Extras I see (all marked with red trash icon) will disappear as soon as I empty the library trash…
Edit: I confirm this was the case!
Are you scanning your library by command line/script? There was a bug in earlier versions of the server which caused local extras to appear as separate movies when the command line scanner was used.
If you have converted your movie library to the Plex Movie agent, make sure to perform a Refresh Metadata for the whole library. The conversion is only finished after doing so.
Until you do, you may see unwanted side effects when upgrading quality and file versions.
No I let Plex handle this automatically although my manual scan I have just triggered was performed via the Server WEB interface
