Okay, so along with occasionally just not recognizing external subtitle files, occasionally randomly pulling the wrong subtitle file, and making the subtitles look like whatever it feels like making them look like, we can now add “detects external subtitle files when you’re in the preplay window, but forgets where they are after you hit play”.
Can you get the Plex Media Info xml for that file

That shows you have 2 copies of the same file in different locations.
one in "X:\Media A [full] and the other in W:\Media C\
Looks like the SRT is associated with the one in media A ( which appears to be the default on the details screen in first screen shot, but the one in Media C I think is actually playing.
Just wanted to confirm you are not using “Play Version”.
