I see the bulk of the problem immediately.
You’re using the “Multimedia” shared folder.
Multimedia is a software API shared folder. It’s not real.
Every time QTS/QuTS scans, it will delete then re-add. You end up with multiple entries just like are being shown.
Solution:
- Add the REAL shared folder (e.g /share/TV) to the definition list
- Let PMS scan it and all those to the pile
- REMOVE the reference to Multimedia.
- Scan again
- Empty Trash
- Clean Bundles.

The entire Multimedia / Codex Pack setup, while good if you’re running standalone (no PMS server), is a disaster if you use PMS.
Use the real shared folder definitions and it will all clear up
