I have this issue too, it’s very frustrating. Like others using a different player (Infuse) works but I’d rather not have to pay for this after the trial.
I’ve tested using old and new player, turning off match frame rate but these do not help.

Is Plex working on a fix for this issue? I’ve been having this issue for months. Tried different servers and made no difference. It’s only on Apple TV’s.

New AppleTV user here, and this issue is also driving me crazy. I’m seriously considering returning it…
EDIT: so in my case the workaround was not messing around the tv settings, but the apple tv sound settings: I changed the Audio Format to Stereo and that worked.
I don’t need more than that for now, so it’s more or less fine. One thing that would be a deal breaker for me was if the solution was to turn off the frame rate match. That’s not an option because that seriously degrades the viewing experience…
Then something must be really wrong with plex, because this only happens using plex app.
I’ve been trying to reproduce this, and I used to be able to reproduce this trivially, but I haven’t been able to reproduce this since v7.22, when we bumped our version of mpv. Is anyone able to reproduce this on v7.22 or higher, and could provide a sample file?
I got my ATV 3 days ago, it was horrible out of sync. Updated to tvOS15 and all is in sync.
