I don’t have an Apple TV, but was thinking about getting one since Plex works fine on my iPad. But I guess TV gets its own share of issues.
Did anyone try installing Plex for Apple TV beta on testflight? In case with iOS you can send feedback to Plex after you make a screenshot, maybe it’s similar with the tv? Might be worth a try to mass-report it there, since no one seems to respond here. Also in testflight you can usually roll back to the oldest available version (but it’s usually limited to 90 days). In case it worked with some recent-ish version for you, you could find when exactly it stopped.
@enchained - i’m running tvOS15 beta and the slowly drifting out of sync audio is still an issue.
In my message I was saying about Plex beta, not tvOS beta. I suppose it’s still broken on any beta, but with testflight there should be another way to send feedback, and a way to roll back to an old version to see if it started on a specific one.
I’ve been running the tvOS TestFlight beta for almost 2 years now. This has been an issue on every single version released in that time period. I’ve sent feedback multiple times.

I am disappointed in the lack of Dev involvement in this thread.
My newly purchased ATV 2021 4K seems to work correctly with the Plex app if I set the output of the ATV to 1080p.
Obviously playing 4K movies at 1080p is less than ideal - but at least they are now watchable. The audio sync at 4K is terrible.
I have frame and range match on in the ATV settings. I use the ‘New’ Plex player, in Plex settings.
One of the hidden perks of buying my new TV was the 120Hz panel so I don’t have to rely on matching framerate through the ATV. I don’t have any confidence this issue will ever get resolved.
I have changed my AV setup, and seem to have fixed the audio sync problems (for my hardware anyway).
I have just enabled eARC on my Receiver and my TV (I was using IR so that the ATV could control my Amp & TV before, and my TV had an optical TOSLINK connection back to the Receiver).
eARC has mandatory lip sync correction, as default.
My setup now:
ATV → Onkyo TX-NR696 -->LG OLED65C9PLA
The ATV control (for Remotes, in Settings) is just set to Auto, so the ATV now uses HDMI-CEC to control the Amp & TV.
There is just 1x HDMI connected to my TV now, no optical TOSLINK – and the Receiver does all the input switching.
I can now play 4K HDR files with almost perfect lip-sync.
It is not perfect (maybe 95% there), but it is usable at last.
I have also just noticed that the Amazon app on ATV can play 4K HDR with Dolby Atmos passed to my receiver
… but Plex can’t.
I just upgraded my plex server to Version 4.61.2, and with the current available version on AppleTV (version 7.20), the audio is now playing IN SYNC!
Now, I also upgraded my AppleTV 4K to tvOS 14.7 (18M60) as well…
Can anyone else verify their media is playing correctly again?
I’ve played several pieces of media that used to be out of sync unless I switched it to “Convert Automatically”. Now when playing “Play Original Quality”, the audio stays in-sync.
Was the issue with the plex server or with tvOS?
Either way, I’m thrilled it’s fixed!
4.61.2 is the web client version. Do you mean you upgraded to today’s PMS release of 1.23.5.4841?
EDIT: And I upgraded to tvOS 14.7 as well, and I think that’s what did the trick. I know this thread is for 4K content, but I was having issues with regular HD/SD content when framerate matching was enabled on the Apple TV. I did a quick test of a movie with AC3 and then one with DTS – they both appear to be working.

Again, not sure if it was the AppleTV client, the plex server update or tvOS updated.
Still seems to be an issue for me. Upgraded to the following:
PMS Version: 1.23.6.4810
AppleTV Plex Version: 7.20
Apple TV 4K (2021) tvOS version: 14.7
TV: LC CX 65" software version: 03.23.06
4K and SD (1080p,720P) content still has audio syncing issues when “match frame rate” setting is on in the Apple TV settings. There are no audio syncing issues if I leave “match frame rate” off. However, it would be nice to be able to leave this setting on. Otherwise, you get 3:2 pulldown on 24fps content, causing judder.
The Infuse app still has no audio sync issues with “match frame rate” on. So, I guess I’ll continue using the Infuse app on AppleTV until Plex can get this sorted.
Yea, it’s not been fixed for me either. I try once per month or so. I wonder why Plex isn’t fixing this. I did move to Infuse for all my viewing though.
I’m seeing the same with 4k HDR. This is really annoying for someone who just moved from Nvidia Shields. I’d love for a Plex employee to chime in here
I didn’t think match frame rate was a big deal on the LG OLEDs. rtings says they do judder-free 24p over 60p.
