Well, since this issue has been reported a long time ago and nothing happenā¦it could be explained by the āsmallā amount of users using plex on apple tv using Homepod ⦠and having this issue. So i dont think this one will get resolved any day now . It sucks ⦠but most of us switched to Infuse and so far so good !
Iām having this issue as well, and hoping for a fix soon. My AppleTV 4K 2021 is connected directly to my Hisence TV which is then connected via ARC to my Marrantz receiver. Some shows are better than others. Most of UK based shows have a different delay to US based shows. May be a 50/60 Hz issue as well?
This issue affects any kind of external audio device (soundbar/AVR/homepod/bluetooth speaker). I dare say that is quite a substantial chunk of the user base.
Itās a shame that no solution has been found by the Plex Team. I too have this issue and I must use Infuse to get it working. I was using the old player for a long time as I have only 1080p m4v files with ac3 audio. But since some iOS update the match frame rate is broken on the old player. So I had to go to the new player.
My wish would be the Plex App interface with the Infuse player.
Just to keep this thread alive - Iāve had sync issues with Plex and the Apple TV for years. The main problem used to be direct playing Blu-ray rips. If Plex transcoded them they were in sync, direct played, not so much.
Now the main problem seems to be that audio is out of sync with anything that has HDR / DV content. Itās always fractions of a second late. No other apps have this problem - Infuse, Apple TV, Netflix all playback without issue. Iāve pretty much given up using the Plex app for 4K HDR content and will use Infuse instead, but my partner canāt be bothered with switching apps. Iāve started using the old player, which seems to fix the audio sync issues but I believe you lose frame rate switching and other improvements that the enhanced player includes.
My setup is the new Apple TV 4K box running the Test Flight version of the Plex app. Itās HDMIād to a LG B6 OLED, which then sends the audio using an optical cable to a Denon AV amp. The LG will pass through digital audio to the amp. I have an Apple M1 Mac mini running the latest beta Plex Media Server app.
If I watch anything on other Apple TV 4K (previous generation) in the bedroom, which is connected to a 1080p TV, the HDR/DV content gets transcoded and plays back without the sync issues.
your post reminds me that I want to setup a test for this using eARC and HDMI w/Ethernet cable.
Optical canāt do any sync adjustment on the fly like HDMI can. So Iād like to see if I can demonstrate that with Plex. Itās about time to get a new Atmos and DV capable AV Receiver anyway.
eARC + Ultra High Speed HDMI cable should be able to eliminate A/V sync issues. Problem is lots of us are still on perfectly usable ARC setups.
Thanks for that info about eARC. I looked at the thread again and saw that Plex can reproduce this and wants to fix it. Because Infuse is reported to fix HDR banding and audio sync issues, I wouldnāt change a perfectly good audio setup either.
Is anyone running HTPC on a Mini, NUC, or SFC instead of using their AppleTV to work around this?
You already mentioned the most common workaround - Infuse. It solves all technical playback problems that ATV4K 2021 has currently.

What is going on here? Everything was fine for me until very recently (last day or 2). Based on this thread and how long it has been going doesnāt leave me hope for a fox any time soon. Is the recommendation for Plex to jump ship to another service?
This issue started for me out of nowhere 2-3 days ago. Anybody know if there was an issue in the latest build?
I can also confirm audio sync issues in the last week. No idea whatās going on, Plex seems to be going backwards at a rate of knots.
Iām also getting this now since about a week. Only on one show currently, but itās very annoying.
Seeing this issue as well with direct playing 4k HDR videos on Apple TV. Audio is slightly ahead of video by a handful of frames. Transcoding seems to fix it but who wants to transcode what you can Direct Play? Starting to consider the Infuse option.
Iām having this issue too now. I just got an LG OLED & there was the Apple TV update at the same time. Now I have out of sync audio in Plex.
Seeing as this thread is 2 years old, I think itās safe to say Plex doesnāt care & is never going to fix it.
That being said, does anyone know a good alternative to Plex?
Iāve primarily noticed this on the 6th generation Apple TV 4K (new remote) - doesnāt matter if itās transcoding, DS, or DP, if itās using MPV the audio is out of sync. If I use the old AVPlayer audio is fine, but large files struggle to play like they always did.
I have noticed the audio out of sync on the 5th generation Apple TV 4K lately too. Itās not nearly as bad as the 6th gen.
On both devices, Infuse + Plex works as expected, and so does every other streaming app. Iām pretty certain itās something in MPV, perhaps thatās exacerbated by the faster A12 processor vs the A10X?

Mine are 5th gen ATV.
Does Infuse do live TV? I have an OTA setup in Plex with an HDHomerun. I added my Plex to Infuse but donāt see live TV. Or is that only in the pro version?
Infuse does not currently handle live TV. Channels, on the other hand, is hands down the best app for using a HDHomerun on Apple platforms!
