Like many others, I have the same issue too.
The issue with me it seems is with the Apple TV 4k device.
I am running a Plex server on a Synology NAS with flawless playback on FireTV (connected to soundbar via optical) and Xbox One, and my iPhone 13. But with Apple TV I am presented with unbearable Audio/Video sync issues. ATV is connected to a recently upgraded Denon AVR (thinking the older Denon was the issue……it wasn’t) via HDMI. I have the same issue whether I use the Plex App or the Infuse App. When I originally purchased ATV4k 2nd gen on release the issue wasn’t present but with later software updates it became prevalent. Turning off Match Frame Rate made the issue more bearable but coupled with slight stuttering as the frames caught up.
In summary I believe the issue lies with AppleTV4k, possibly due to not having the ability to enable audio passthrough which is causing ATV to transcode everything. I think my only option as of now might be to purchase a Shield device which is a shame because I’m a huge fan of iOS.
I shall look into joining Infuse beta and see what comes of it. It’s pretty infuriating that the only place it doesn’t work is in my home theatre room where we do most of our viewing.
Oh I agree. Running a Plex server where people have access to and telling you that the crap you have on your Plex server doesn’t work is annoying. Have to get 1080p lower bitrate files for my non infuse/nvidia shied/fire stick users out there.
I have a emby and Jellyfin server to play with/back up as I want to move away from Plex and emby has no issues with the same files. Jellyfin will even transcode DV if the user doesn’t support it (as long as it’s a hybrid file). Plex. Hard no.
I’ve been playing around with the settings and I found out that when I set Audio Format to Dolby Digital 5.1, the audio is in sync.
Not the solution I was hoping for, but at least I can watch properly until there is a real fix

It would be really helpful if the Plex team could confirm which issue they are looking into and also when people reply could they mention which issue any workaround they found is addressing.
This is super frustrating as I only noticed it starting with one of the newest tvos versions. But one I found can help get audio and video a lot closer to being in sync is this:
Open the video you want to play on Apple TV. Then take out your iphone open Plex and AirPlay a video (maybe just screen mirror will work, haven’t tested). Once it’s played for a few seconds, you can stop airplaying and the video in Plex on tvos is much closer to synced.
This is not a fix and super annoying and frustrating. Especially since I just ditched Amazon fire sticks all over my house for Apple TV 4Ks now that they are on sale for 99$.
Just chiming in to say I also have video lag issue and it’s lame af. Using Infuse for now. It’s very noticable and makes it unusable. Can the Plex team explain the root cause of the issue? Maybe the forum can crowd source a fix
I experienced this issue, but it turns out I was able to fix it by changing the way I routed my audio over HDMI.
Lag Setup
Apple TV > Soundbar > TV
Lag-free Setup
Apple TV > TV > (via ARC) Soundbar
Conclusion
It seems like my soundbar was contributing to the lag. When I switched to connecting the Apple TV directly to my TV and passing the audio to the soundbar via ARC over HDMI, this solved my lag.
My setup is a bit older; I have a Panasonic plasma TV and a Sony soundbar. I saw several reports online of the Sony soundbar being a problem with lag, which led me to this new setup.
Using old player did help for me, but not for everything. Transcoding DTS MA to AAC still produces significant lag. Hoping for fixed APIs in 16.2. Having to manually adjust my audio delay for Netflix/Plex direct play/Plex transcode is driving me nuts
One of these days the Plex app on your iPhone will adjust the sync when you point it at your ATV.
Just like they can put up a debug overlay, they can flash something and make a beep at the same time, gauging the offset instantly for that hardware & remux. Feedback the offset to the ATV API if that’s possible on the fly, and there you go.
It might be something they do for a hack-a-thon competition among the devs.

Hello folks! I’m new to this thread so bear with me.
I’ve been having audio sync issues on my ATV 4K for a while now. I wanted to report that I just installed the latest TestFlight version as of today, and the sync problem appears to be gone, or at least, not noticeable anymore.
Has anyone else with the problem had the same experience? The version I installed was 8.12.1.
I just tried using latest Testflight Plex and the 4K HDR audio sync issue is most definitely still there. I switched back to Infuse and the audio sync is PERFECT. Tried several movies. Plex is still a broken mess for 4K HDR content.
I am going to try Jellyfin instead of Plex Server. I know you still have to use Infuse as there is no Apple TV native client but I am just fed up with Plex. It feels like all their focus is on the streaming TV/movies stuff rather than making the core functionality work correctly. Enough is enough.
