Ok i already know that there are various alternatives but with previous Plex version if I plugged Apple TV in AVR or Tv directly I had the same issue.
So something is changed now for me!
Well I have to admit to using Infuse with my Plex server to actually WATCH content. The Plex app UI, and features are WAY better, but it’s fundamentally broken for me (and many others) in its current state.
PLEX!!! Love you, but you’ve got to address this problem with audio sync issues with your Apple TV app. PLEASE. Silence suggests the problem is fixable, which it isn’t. At least say something about it. Tell us you know about it and you’re working on it. Something. Anything.
Just as a little data point, the most recent TestFlight builds are a big improvement for me on 4k Apple TV 1st and 2nd gen. One thing I have noticed is that after a day or so use, the sync gets worse. But restarting the Apple TV fixes it so I wonder if there is a subtle memory leak happening?
What would be really helpful in these posts is to also include the Apple TV 4K current video setting under “Video and Audio” as this is another hot topic.

For those of you who are using their AV receiver to fix their audio sync issue, how many milliseconds did you have to set yours to sync your audio? This may be useful for other users as well as the Plex engineers.
Wanted to share one other non-Plex app that I’ve seen similar audio delay on Apple TV; the YouTube app. I have an LG CX OLED which has an AppleTV, Xbox Series X, PS5, and latest version AppleTV 4K plugged into it (with a Sonos Arc for audio). The AppleTV is set to Dolby Vision 4k, and matching Dynamic Range (ON), but not Framerate (OFF).
When I launch the YouTube app with these settings and then select the latest Late Show with Stephen Colbert episode, I always see a black flash before the video plays, and occasionally the audio sync is screwed up. I back out, relaunch the clip, and audio sync is fine.
I recognize YouTube is not Plex. Just sharing in case there’s some hint here that can help.
