Video issues like that are not related to AV Sync, and if there are issues like that, I’m keen to see a sample file, and see what the issue is, if the file isn’t muxed or interleaved properly, or if there are other issues with it, or if it’s to do with the player itself.
I am also seeing dropped frames with the current beta 8.18 (5743) on the Apple TV 4K 2022. It does not matter if I have the audio offset to 0 or something else. If I revert the player to the current version of the app, the dropped frames go away. I’ve been able to reproduce on multiple files. Dropped frames with new beta. No dropped frames with current release version. Also, the dropped frames don’t always occur at the same moment each time. I would be very surprised if it’s the file, but I’ve provided a sample file…

Yes it helps, because you suggested the problem could be the media itself (muxed or interleaved in the wrong way). If every player in the universe plays the file correctly and Plex plays it wrongly, maybe the problem is Plex.
The report was to do with differences in our app versions, so reporting on a different app does not help that issue
It’s meaningful because it shows the file / TV / AV equipment etc all work fine when using a different stack so it rules out quite a lot so shrugging for a problem that’s been around forever really isn’t meaningful or helpful.
You/Plex we’re supposed to do the investigating. Not me. I’m not going to do the QA for a multi million dollar company that bans their paying users from their support forums begging for help on a 3 1/2 year issue

Not only do I have the audio sync issue, but version 1.32.0.6918 made my plex server appear remote and thus made it impossible to use with VLC or Infuse while on my HOME local network. Had to revert back to previous version of PMS to fix the issue. Plex is struggling here…
It looks like this issue has been pretty solidly identified but I figured I would throw my name in the hat of lifetime members struggling with A/V sync issues on an Apple TV 4k. I haven’t been able to figure out how to get the audio offset feature to show up for me, but hopefully it’ll go to production soon. Looking forward to any fix for the time being, but fingers crossed that a more sophisticated one follows.

To add another data point, I’m a new user of Plex and I’m also suffering from this problem on my Apple TV 4K (2nd Gen). Like others have mentioned, enabling Match Frame Rate fixes the “jerkiness”; however that introduces audio sync issues. I would update to Apple TV 4K (3rd Gen) but I doubt it will solve the problem.
I’ve seem to workaround the jerkiness and audio sync issues by disabling Direct Play. I force transcoding of my 4K Remux sources to 4K “High”. My Plex server has the CPU “horsepower” to do this. Obviously, this workaround still has caveats as quality is inferior to Direct Play.
My girlfriend hates the Kodi interface and loves the Plex interface (duh). She’ll tolerate the quality compromises to use Plex. Because I’m more particular with movie quality, I’ve been using my OSMC Vero 4K+ (Kodi) to play movies on local network. But obviously, the Kodi interface sucks (compared to Plex).
My Setup:
- Apple TV 4K (2nd Gen)
- LG OLED CX77 TV
- Denon AVR-X3700H Receiver
- Apple TV is hooked up to the Denon, which is hooked up to the LG TV HDMI ARC
I don’t have a 2022 Apple TV 4K, but I do have the 2017, and 2021 models, and I’ve tested your sample in its entirety on both with 8.18 (5743) and 8.17 (5542), and haven’t been able to replicate any issues with dropped frames at all, I’m afraid.
Thanks for trying. Not sure why its’s happening. I’ve reverted from the beta back to the current production release and haven’t seen the problem. I’ll try again when the next version is released.
Infuse for Apple TV. Works flawlessly. It can connect to your Plex server. I am using this until Plex fixes (if ever) this issue.

Anyway, tks for your job because with the last official Plex release, and switching on audio lip sync on my AVR Yamaha RX-V4A, the audio issue is solved!!! Maybe (strangely for me), a lucky strike … (in Italiano… una botta di culo)
So I’m afraid for updating and I’m thinking I’ll remain on this version forever!!! I’ll remain on the same firmware on all my device, also for my domotica

For local network movie watching, I’ve been using my OSMC Vero 4K+ (Kodi). I use the Plex app to “discover” what I want to watch, then change source to my Kodi device and look for the same movie. That’s my personal “workaround” until this gets solved. ![]()
For me, the jerkiness and audio sync only happens with Direct Play. When I force transcoding, I don’t think I’m encountering these issues. When I search for “transcoding” in this thread, I’m seeing mixed observations on Direct Play vs Transcoding.
Question: Is this only impacting Direct Play? If so, I’ll force transcoding until Plex fixes this. ![]()
I’m seeing something similar on two AppleTV 4Ks (both 3rd gen) and basically every 1080p and 4K Blu-ray rip I tried (all ripped myself in MakeMKV). Videos seem to drop a frame every 10-25 seconds or so. I wouldn’t describe my experience as stuttering, and definitely not network related. Played normally before, and play just fine in Infuse as well. Just started in 8.18.
