For those still following this…this solution appears to have helped, for now.

[Bug] Random audio out of sync issues when Roku is set to automatically adjust refresh rate

Server Version#: 1.18.3.2129 Player Version#: 6.4.9.6153-6d1b63c9b If Roku is set to Auto Adjust Display Refresh Rate under advanced settings (this is necessary if you want things like camera pans to display smooth, not all choppy – not sure how anyone could stand it any other way), often the video playing in Plex will not have audio synced correctly, requiring you to skip ahead/back to get it to sync back up. Netflix and most other players on Roku do not have this issue.
