Steps to replicate:
Add a home user, protect main profile with PIN.
Perform clean install of iOS app (specifically I tried on iPad).
The initial setup screen shows, presenting only the main profile and not requiring a PIN to access the data within that profile.
Only when explicitly changing profiles afterwards, the app will start asking a PIN for the PIN protected profile.

That’s completely normal and expected (as you’re already logging in with your main credentials).
Do you have “Automatically Sign In” enabled in the Settings → Account?
Automatic sign-in is disabled. It seems to request a PIN each time on any other platform. I guess indeed inviting external accounts as home users may fix the issue in this case. Still, weird that it’s not asking a PIN on initial setup.
Thanks I’ll ask somebody to double check what the behaviour should be.
