100% agree on the automatic scans. I’ve found it’s much better to initiate the scans when I want but understand that’s not for everyone.

If the file persists despite doing the ENTIRE Plex Dance , then click the triple dot and Get Info on it. Look for the full path to the file that Plex thinks it is. Cleaning bundles removes cached data bundles that could be reused after adding things back. I see you tried to remove and re-add but think you missed some steps from the plex dance which should cleanly rescan and scrap the file.
