I had an old server that has been dead for a long time and the device has been unauthorized.
The server does not appear in the web app however the server shows up on iOS, iPad, and on TV apps. not sure how to remove this bugged out server as the web portal does not allow me to access it in any way.

Tanks Otto for replying! It’s an old server of mine that’s been long removed.
There is no old server in your account.
So this must be only a caching issue on the affected clients.
Verify that these clients are actually signed in to the correct Plex account.
Reboot these devices.
In some extreme cases you might have to sign out these devices, reboot them, then sign them in again to your plex account.

Didn’t realize on apple devices the data transfers over as you upgrade devices (including cache or other settings for Plex)
Logging out and back in has resolved it! Same with the TV. Should have tried that to begin with before bugging you
