I assume you have enabled ad detection and activated it for the library in question?

The Plex settings for comskip only seem to work well in the U.S., for any other country, it will burn your CPU trying to detect ads after the recording completes, and then either give you a bunch of junk that doesn’t skip correctly, or no skipping at all (i.e. it seems like it never ran).
I have disabled ad skipping for any content other than the U.S. because it just burns your CPU for no reason.
I stand corrected on the country. Comskip does seem to work for other countries, but Plex only does it automatically for items that it records using its own recording system. If you want comskip to be run on any other items than the ones Plex records, you have to manually choose “Analyze” after already setting ad skip “for all items”. It becomes painful to do this manually every time something new is added to the library that wasn’t recorded by Plex; hopefully this will become automated at some point.
