So I’ve been experiencing the same problem for a while now and it was getting progressively worse until it was every show - I had reached out to SiliconDust for some assistance and they were telling me that they were seeing some packet loss and that it was causing the issue. They gave me some tips and it maybe helped a little. I reached out to Comcast as well once all recordings started to fail and they actually found 3 issues with the cable connection into my house.
And that made sense that was the issue as I’ve been running Plex for well over a year with really no issue . . .
So Comcast does their fix and I’m like ok . . .we are back in business! And sure enough stuff starts recording again with no problem . . . for a day or two and then I start getting a single recording failing. I notice it starts pretty much on the hour as a new recording is starting - I will typically have 3 to 6 recordings going at once.
So I found this thread but before I posted I wanted to do some more testing as I didn’t believe it was Plex that was the issue and was instead a firmware update from SiliconDust.
So I switched all of my recordings over to Emby for the past week and a half . . . and all of my recordings have worked . . .
Really annoys me to say that as I DO NOT like Emby - don’t like the interface, don’t like how scheduled shows are displayed, don’t like that I can’t prioritize shows, don’t like that I can’t see if a show failed to record, I can’t run it as a service (this I will be putting a post up for as when I do run it as a service it’s not finding my WDCloudEX4100), don’t like that you have to click the show on the right channel (or that you can’t prefer the HD recording over the SD)
But I’m going to use Emby until I get confirmation here that the issue is fixed . . .
My setup is simple - I have a Win10 computer and I’m the only one that uses it - I run exclusively on Apple products - server is wired and not wireless. I’ve done a lot of tweaking and tuning over the past year or so and it’s been running smooth until recently. I run MCEBuddy post processing to remove commercials and convert to hevc 265 to save space.
tldr - if you have this issue switch to Emby as it works - annoying as that is.
I wanted to report back since I started rebooting my router 2 or 3 times a week - I haven’t had any more glitches. The router settings I made in this post DVR "Refreshing guide data" stuck in "Initializing" for days fixed the guide data issue and most likely helped with keeping things in general running smooth. I also like to keep my firmware up to date on my router and I’m thankful for having Asuswrt-Merlin who takes the time to do that. If you are having issues- make sure you have a quality router that has the ability/settings for proper configuration. I really like the Asus TM-AC1900 because you can buy it really inexpensive (used for $20 to $40) and then flash it to a RT-AC68U. This gives you the ability to use the Asuswrt-Merlin firmware. Whatever router you get- just make sure it’s something that is updated frequently especially with a custom firmware. To give you an idea of how many things are changed on a constant basis - look at this changelog https://www.asuswrt-merlin.net/changelog-382 If your router doesn’t have constant changes like this to keep up with the times- consider getting a different one.

Just a little update from my side. I tried downgrading to version 1.16.1.1246 it didn’t work so I tried version 1.16.0 1226 it didn’t work either. So I tried upgrading to the beat version 1.16.5.1488 and now everything thing seems to be working just fine. Fingers crossed, I will keep monitoring if the problem come back.
