Enabling subtitles on 4K HEVC video when hardware transcoding on Synology DS918+ gives slow server

If I enable subtitles when watching a 4K HEVC video being transcoded, playback stalls, then plays in small bursts between buffers after warning of in sufficient server speed.

The CPU is barely being touched by this however, and it looks like a bug when baking in the subtitles for the transcode.

This is happening on Plex Server 1.11.1.4760 (64bit), on a Synology DS918+ NAS.

Tried it on different clients as well same issue or video will not play at all

Interestingly, Emby Media Server also has this problem.
