Your help is much appreciated. I had this working perfectly yesterday but i'm getting same issues as guys above. Details below:
Your ATV model (2 or 3) and firmware version (found under the ATV settings > general > about > Apple TV software): ATV3 (5.3/ 6105)
The DNS server set on the ATV (found under the ATV settings > general > network > DNS): 192.168.100.4
The device and operating system (including version number) that PlexConnect is installed on: Mac OS X10.8.4
The local IP address of the device that PlexConnect is installed on: 192.168.100.4
The device and operating system (including version number) that the Plex media server (PMS) is installed on: Mac OS X10.8.4
The Plex media server (PMS) version number you are running (**do not put 'latest'**): 0.9.8.3
The local IP address of the device that the Plex media server (PMS) is installed on: 192.168.100.4
The PlexConnect version number. If using Github source then a time and date of download (look at the creation date of the folder) and if you are using any testing branch e.g. the Elan/PlexInc one. Downloaded using Git 31/07/13 12:50pm (GMT +3)
The content of your 'Settings.cfg' file (if you do not have this file you are running an old version, please update)
Detail on the media file if it is an issue related to playing a media file (in order to get the necessary information on a particular media file go into the Plex web interface, select the video file, when it brings up the page for the media file click on the "i" information button on the left hand side, it will then bring up a "media info" box, click on the "View XML" button, it will then open a browser window with the relevant information, copy this to http://www.pastebin.com and use the pastebin link to post the details; posting the information directly in the forum may mess up the formatting and generally takes up a lot of space):

I read through this thread first and changed the Settings.cfg file to hardcode the IP_PMS. I've also tried changing value of ENABLE_PlexGDM to False and disabling it in PMS settings. I cannot connect to the PMS in the ATV or it returns an error saying cannot find UUID of PMS. With standard settings, either with hardcoded IP or not, I get an error on ATV when I try to play the file. I had Elan's fork installed before this main version and it came back with same error, giving a "Broken Pipe" error on logging in Terminal. I imagine the same is happening here.

Ensure you post your 'PlexConnect.log' file to pastebin and post the link ('PlexConnect.log' is located in your PlexConnect folder). If possible, follow these steps to create a "more detailed fresh" one: location is here https://dl.dropboxusercontent.com/u/16186677/plexerror/PlexConnect.log

So far i noticed the AVI's are giving the most issues.. MPG formats are OK
It is interesting you ask that. I am running Vmware on my Win 7 PC which also happens to run PlexConnect and PMS. Is there any known issues with this? Thanks.

There can be as it can confuse PMS auto discovery (as it either NATs or bridges the virtual machines IP).
Try disabling GDM and hard funding the PMS IP in settings.cfg.

Auto discovery works fine for me (with an older version of PlexConnect though) and Plex works on my ATV3 (although I have a feeling media playback is not 100% perfect. I haven't had much of a chance to test it completely). My settings.cfg does not have the correct IP of my PMS (I believe it has 192.168.178.10 which I am guessing that's the virtual interface?). Do you think if I hard code the PMS IP in settings.cfg, things will improve even though Plex stil works with the current settings? Thanks.
Check the Plex wiki on your NAS device to see if it able to transcode the problem files.
Hi.. your right.. qnap can't transcode.. issue found :) looks like ill have a mac mini sitting in the server room to be the PMS and PlexConnect and it can access the files off the qnap..
Quick question, how many streams can PMS transcode simultaneously? would 3 be OK. I am using this in a large home theatre system that uses a central control system with 4 apple TV's and there is a chance that all 4 apple TVs will want to watch something from PLEX at the same time...
Any foreseen issues using the i5 iMac mini as a PMS?
The how many streams question is dependant upon PMS hardware and client codec support e.g. sending multiple streams doesnt require too beefy hardware, direct streaming requires a little more effort and full transcoding requires even more, PMS itself will easily handle multiple streams.
I would think that your Mini should be able to handle it but maybe worth checking in the PMS forum (provide the full specs though including CPU speed and RAM).
