I have resolved this. I basically went to C:\Users\USERNAME\AppData\Local\Plex Media Server\Plug-in Support\Databases and renamed all three files that started with “tv.plex.providers.epg.cloud”, there was a .db, .db-shm, and .db-wal. Once I renamed it and started plex again, all the sudden the HDHomerun was back with the original settings and guide data refreshed.
