One thing to remember when working with an ISP, setting up things like port forwarding you are typically using some kind of HMI/API and not directly changing it in their servers. This can take up to 3 days to migrate (a rough guess on my part). I have Comcast in the States and have seen port forwards I created take over a day to update.

The router behind the ONR is 198.162.1.1 as you mention and I have another port forwarding rule pointing to the NAS (10.0.4.23). The ONR is in router mode not bridge and I’m worried that if I change it to bridge I might not be able to access my network remotely.
no, you don’t forward the port on the ONR to the synology router, you forward it direct to the plex server
you should have 1 of the devices running in bridge mode, otherwise you’re running double NAT.
the preferrable option would be the ONR in bridge mode and the synology router in router mode, but you can do it the opposite way round if required.
you should really be sitting in front of them though when doing it.
as it stands right now, all your port forwarding rules should be done on the ONR as that’s your primary router with the WAN IP. the synology router shouldn’t have any port forwarding rules as it’s not visible to the WAN.
I can’t point the ONR directly to plex as the drop down selector does not allow me to put in an IP in the range 10.0.x.xx. I presume this is the double NAT situation.
Like you said I need to be sat in front of it to do anything or risk cocking it all up.
Just out of my curiosity why can’t I point the port on the ONR to the router and have the same port on the router pointed to Plex, that’s what I have now and it doesn’t work but would like to understand why.
the synology router doesn’t have any services running on port 32400 so the ONR has nothing to forward it to, hence it reports as being closed.
what’s the “unknown” device, could that be your plex server?
Honestly not sure what it is as it’s on 192.0.1.168 and all my devices are dhcp from the Synology router on 10.0.x.xx so it’s definitely not the Plex server.
you need to get your LAN side network sorted out, then your port forwarding will no doubt work correctly.
but you can’t forward port 32400 to the synology router, it needs to be forwarded to the IP of the plex server itself.

i already answered that.
there is no service running on port 32400 on the synology router, so there is nothing to forward the port to. you need to forward the port to the end point, which is the plex server.
