When I am using PMS on my local network all of the streaming quality options are there. However, outside of my network for any file, it gives 240p or 328p, how can I get these clients to play my files at the original quality?
So you cannot really get 1080 outside of the network even with plex pass and relay since the max stream is 2 mbps?
The main reason is that your plex server at home is not directly reachable.

Do I need to setup an inbound firewall rule for the port on the windows based PMS?
No, the setup routine will do this automatically on installation.
(at least if you use the default Windows firewall and not some 3rd party product)
Would you suggest using 32400 as the manual port in Plex or something else?
Usually recommend to use a different, random port number.
Keep in mind that only the external port can be different from 32400.
Everything works for about 5 minutes and then will say not available outside your network. I do retry with the port forwarding and then it will work again but then cycle through that process. How can I get it to stay?
Did you set the checkbox in the Plex user interface?
If you did, just let it sit for a few hours without changing anything.
