Hi,
Until recently I had a show that showed all seasons and all episodes.
For some reason recently season 8 lost all but 1 episode.
I checked the file names and all have the same format so no reason it seems for why only one is showing - I can’t find any way to find the missing episodes elsewhere in Plex Media Manager - could someone help?

Have you tried doing a Deep Scan? If that doesn’t work, please give us specifics as to the exact filenames you have and the directory structure you use so we can test.

I am also having this issue, I have tried all the different types of scans but it hasn’t helped. So far I think this is only happening for one series but I haven’t checked them all.
Here is my naming convention:

3x13 - Chuck Versus the Other Guy

I have tried the naming convention S03E06 too.
You'll need to name your media as per the naming guide to get the best results. :)

All miss the episode, as far as I can tell I haven't missed an episode anywhere else with my naming conventions.
I am also seeing this behavior using PLEX Media Server 0.9.5.0-dc1de23.
I first noticed that in
TV > Ninja Warrior > Season 18
the files
Ninja Warrior - S18E01 - PDTV x264 W4F.mp4
and
Ninja Warrior - S18E04 - PDTV x264 W4F.mp4
were recognized (i.e. listed when I run Plex) but
Ninja Warrior - S18E02 - PDTV x264 W4F.mp4
and
Ninja Warrior - S18E03 - PDTV x264 W4F.mp4
were not.
On further inspection, I discovered that seasons 1-10 only had 1 episode recognized even though there are multiple files in the directories.
I recently moved from an older version of Plex 0.9 where the server was not separate from the client. In that version seasons 1 - 10 had all episodes recognized. Something happened when I transitioned to the new version of Plex Media Server so that episodes 1 - 10 no longer are recognized as they were. Further, new episodes aren’t always recognized either.
This isn’t a consistent problem. Many other TV shows work as expected.
I had a similar issue with one episode of Phoenix Nights not being seen, in the end I used Handbrake to convert it from .mkv to .mp4 and it then appeared, despite the fact that I had ripped all the other episodes into .mkv.
I actually ran in to the same issue with the same episode. Just renaming the file and removing the word "Sampler" worked for me. Maybe Plex has issues with indexing files with the word Sample in them? That was my guess. Haven't checked deeper though as it works now with the word removal.
I had exactly the same problem with the same episode of Chuck, and renaming the file (removing “sampler”) worked as well. I had the same problem with Boxee too btw!
Plex does in fact ignore files with “sample” in the name. Many torrents come with a sample files you can download with just a minute or two, to see the quality before downloading a whole movie which could be a large file
Apologies, I should have put my fix up here a long time ago!
In my case the issue was that certain folders that had been copied using a different account were locked with admin privileges meaning that Plex could not read them.
Simply select the top folder, correct the permissions and choose ‘Apply to enclosed items’, you will need the admin password to correct it.
Took me a long time to realise that, hopefully this will help someone reach that conclusion faster!
just ran into a similar problem.
Fix for me was to use a Unix type ‘touch’ command in Windows to update timestamps for each missing episode.
From the command line pointing at the dir containing your files
copy filename.ext+
or use +,,

I imagine simply doing a [Deep Scan](http://wiki.plexapp.com/index.php/Frequently_Asked_Questions#What_do_the_different_types_of_scans.2Frefreshes_do.3F) probably would have worked, too, since that bypasses the scanner checking directory mtime (modification time) info and skipping directories that don't appear to have changed.
My solution was to get rid of spaces in the names when using an OS X version of the server, despite spaces in the show name not being a problem in Windows Plex Media Server.
i.e.
Its.Always.Sunny.in.Philadelphia s02e01
rather than
Its Always Sunny in Philadelphia s02e01
I’m assuming its the OS X version lagging.

Episode name wasn’t needed, but it was in the file so i left it.
