The required questions on the form don’t have help with how to answer. Like provide a url to official channel lineup. What would this be for an OTA lineup where each channel is their own provider? Examples next to questions are all cable provider examples, which does not help.
This means a link to the webpage that shows the correct EPG information.
Provider name: Select OTA if you are using an HDHomerun, for example. OTA means “Over the Air” as in traditional aerial based TV.
Line up name: Answer this if your previous answer was “other”.

It does help. I see there were also some updates to the form regarding “Link to Official Lineup” and now has local guide example "Plex web. e.g. “Local Broadcast Listings (151)”
Although, I am confused by what is meant by plex web and where the (151) would come from in the example. I can type “Local Broadcast Listing” but have no idea how to tie that to a particular broadcast marker, which is what the ‘151’ seem to imply. Or, it could be related to the DVR tuner settings which mine shows as “Local Broadcast Listings (41 channels)”, however that is not going to help with where those 41 channels are and more importantly the 42nd channel that is missing from the EPG.
