Playback error: Conversion failed. The transcoder failed to start up

I have this problem, which I have had for many years. First time I reported it was 2017 Subtitles ruining playback.
then again 2020 Falsely displaying duplicate subtitles, one working and one crashing the transcoder on play. I get these “ghost” subtitles that exist as available subtitle in plex but when I check in the hard drive folder its not there. I think every time this have happened there is another subtitle with same name and language, so it’s like the ghost subtitle is a false duplicate of an existing subtitle. I know manually updating metadata solves the issue for this perticular item, but this anomaly keeps popping up in my library items and I can’t keep track on where its going to happen next. I would say 15% of my items, both TV-shows and movie libraries have these ghost subtitles. The problem is that when a player tries to play this ghost subtitle it crashes the trascoder and the message “Conversion failed. The transcoder failed to start up.” pops up. Me myself know the routine, pick another subtitle, but my frinds and family don’t. I keep hearing that they like plex but it crashes too often when they are about to start playing. I have so much library items that making a full refresh of metadata takes long time and not prefered to do at a weekly basis.

I curious why the setting “Refresh local metadata every three days” is not fixing this issue.
