It might be that that Plex can throw this exception for many reasons. So maybe the Plex devs are glossing over this thread because they think we are reporting many different unconnected bugs.
But that’s not the case. Everyone in this thread is experiencing the same very specific problem. After resuming from sleep (especially long sleep), there’s a specific error: WARN [PlaybackSessionController] we failed to create a playable URL! leads to ERROR Playback error occurred. Reason: FailedToCreatePlayable, Failed to create a playable with the current play queue item! Message: [existsOrThrow] Value is undefined or null! It seems like this problem occurs on many flavors of players, from Desktop to SmartTV.
I really don’t know who at Plex is monitoring the Players forum, or whom to @ for this. @gbooker02 ?
Just adding my name to the list for this issue as well. I’ve been experiencing the “[existsOrThrow]” error for probably 6 months now and it occurs almost daily. While it does seem to occur more frequently after bringing my HTPC out of sleep, I have experienced it after it’s been awake for a while, and even after playing some content. When I experience this issue, content is still playable on an Android device.
I’m also experiencing the issue as described by dancb2 where the UI breaks. The UI isn’t frozen and you can navigate the side menu, but no content displays. Restarting Plex HTPC seems to temporarily resolve both issues.
Player is Windows Plex HTPC, and server is running on a QNAP NAS.
I suspect another ‘trigger’ for getting this error message may be the deletion of a media file in Windows explorer that is currently included in the Plex library (not trying to playback the deleted file though) and/or updating the Plex Media Server library.
I can confirm the above to a degree, I attempted to playback the next episode in a TV series after I had deleted the previous episode and this error was thrown. However I was able to successfully start playback of a movie.
My Plex client usually has “Offline” status after waking up from standby. I usually reconnect to my local Plex server in order to get the “Online” status, but this time I chose not to reconnect and kept it “Offline” and to my surprise, I could continue watching the remaining episodes without getting the dreaded playback error.
I haven’t been able to test this “offline” scenario more than once, but if anyone else could try and check if this works, please report back.

I also am always in offline mode (Plex HTPC should automatically reconnect after resume from sleep / internet availability)
And also habitually manually reconnect.
Has anyone from Plex support responded to this yet? I’m still seeing this error pretty much daily. It does not seem dependent on bringing the HTPC out of sleep, as I will see the error well after waking the PC up.
Also, there hasn’t been any update for Plex HTPC in a while… curious what is going on… Maybe they are preparing for something big? Where this issue is also fixed?
I have this issue on a new Samsung TV - are there any solutions or hot-fixes?
Just wanted to mention that I have this same issue on a Samsung TV (2019 Frame model). Same as others have mentioned, I can go to another episode and skip back or forward to the episode I want to watch to circumvent the issue, but would like to see a fix for this soon. Surprised to see how far back people seem to be reporting this same issue.
I guess we have to wait until the New Plex Experience arrives… It’s a rewrite of the existing apps… hopefully those issues will be fixed then…
Jumping in here, also have this issue regularly on LG WebOS Plex app.
Resuming a partially watched episode, or starting the next episode in a series after being idle more than a few minutes seems to trigger it. Starting a different show for a few seconds and then going back to what I wanted to watch works. Even restarting the episode I had just watched, and clicking next, often works.
Very strange. Hope it can be fixed sooner rather than later.

Friend of mine is using a Samsung TV, and is also having this same issue. Any idea what this could be? Plex server is an Ubuntu 24.04 machine running the latest Plex Media Server.
Also have this issues with a Samsung TV, seems to occur mostly when next eiposde has been partially played before. Exiting the app and going back in will then restart the episode and play normally. Had this issue for about 8 months now.
Fwiw, I have the issue running Plex server on a Mac with Freestyle 2. But Samsung Q7 is ok. When running on my Windows box both are fine.
The isuue is still there on LG TV.
