Yes I did complete uninstall, and manually removed and verified /var/lib/plexmediaserver was gone.
Re-installed and set everything up from scratch.

Interesting - have you checked the contents of that table to make sure it has only the dvrās that you would expect it to? It could be your issue is caused by a different problem that just happens to show the same error message.
For me - the deletion of those old deleted dvr records really has made a huge difference. So far I have not gotten a single one of those errors - and for example last night - I recorded 7 different shows spanning up to 3 simultaneous tuners - again with no issue.
Hope you are able to pinpoint the cause of your error soon - I know all too well how frustrating this problem can be - always shows up at the wrong time.
Yeah a few posts ago, I commented that I removed some old DVRās from the table to try that and still got the error after some time.
Basically I was willing to try anything and was hoping starting over would help, however it did not.
I have had this issue of one of two HD channels here in the UK come up with this error, I can sometimes rectify it by forcing my Android phone to tune the channel to kick the tuner back in for those channels, but most of the time I just have to reboot the system, even on Linux to make everything work again.
I also tried a remove and re-do the DVR and have also reformatted and installed everything fresh and it happened today on BBC News HD, so I rebooted the system and all came back up nicely, but I know this shouldnāt happen like the issue Iām currently getting Live TV through the browser and recordings or other users watching TV tripping out the tuner with a Playback Error. I do hope both issues can be fixed soon though

Dear Plex,
+1
I have been dealing with this issue since I started with Plex in March 2018. Please put some time and resources towards fixing this. Thanks.
Lou

Is it primarily this tuner thatās giving people issues? I have the same, and regardless of the viewing device (Android TV/Roku), I physically cringe whenever I click on a channel to start liveTV, knowing itās probably going to have about a 30% success rate. If it doesnāt start up after trying 5 times or so I have to restart the plex server to get it to work.
The sad thing is it looks like I posted in this same thread on January 1st, and Iām not sure if thereās really been any improvement. With a 4 tuner card (and generally only 1 user), you would think Iād have better success just trying to watch live TV
@CAPT_Lou @jayoungers have you upgraded to 1.14? I use same card and same OS and since I installed Iāve had zero issues tuning or recording.
Not yet. 1.13.9.5456. I run Plex in a Docker container so I have to wait until that gets updated.
I updated yesterday and am cautiously optimistic: so far I havenāt run into any issues.
On our Shield TV anytime I back out of channel or change the channel, thereās a āLive TV recording failedā alert, but it doesnāt impact anything
Yes - I was seeing that last night as well when exiting a show I was viewing live. And as you stated - didnāt seem to affect any of my scheduled recordings.
I am having a tuning issue with my apple TV 4th gen. I am running the latest version of apple tv tvOS 12.1. The issue is a bit hard to explain and am hoping that the attached logs will help. The version of server is currently 1.13.9.5439. I reinstalled this version after having installed 1.14.0.5465 to see if that was the issue. reinstalling the older version did not fix the issue. It appears as though, when I attempt to tune into a channel, the apple tv grabs a tuner (for lack of better explanation) but does not release the tuner after the apple tv freezes. If I do this more than once in a row, the plex server sees all tuners of my Hauppauge quad as taken. The only way to release the tuners is to restart the plex server. Please let me know if there is any additional information that you may need from me.
UPDATE: I have also attached crash logs as my server now crashes when attempting to tune channels. The attached photo is what my apple tv looks like when attempting to tune a channel. if I move the timeline indicator to the left into the solid white part of the line, the channel will eventually tune in. (see photo)

This update did not seem to repair the issues that I was seeing with my Hauppauge quad hd and my apple tv. I have explained this in this discussion in detail with logs attached. I hope this helps to figure out the problem.
Iām having the same issues with a WD My Cloud Homeā¦
@sa2000, Evidently there are still issues. Iām running 1.14.0.5465 and was recording āSEAL Teamā at 8:00 pm. I tried tuning that channel around 8:08 pm on Plex Web and got āUnable to Tune (no parts).ā Tried tuning a sub channel 5.2 and couldnāt load that either. Also tried tuning on Android client and that also didnāt work. Basically it now shows four active recordings as pictured, when there should only be 1. Iām sure restarting PMS will clear it out, however I canāt do that until the show finishes recording. Not a big deal as it appears the show is indeed recording, but I wanted to make you aware, there are still issues.

What streaming devices are you using? What you have described here is exactly what I am experiencing and I have the quadhd by Hauppauge as well. At this point in time, Tuning into live tv for my just does not work. it worked great for a long time with no issue and now nothing but problems. Whatās frustrating is that logs have been provided yet no acknowledgment from plex that there is a problem. I am paying for this feature but its almost completely worthless for me. I have provided logs and in depth descriptions, but feel this issue, which is affecting many, is not getting taken care of.
Exiting the plex server and launching it again does clear those āphantomā recordingsā.
I think your roll back version is too new. 1.13.8.5395 and newer are affected. I use the same card, try rolling back to 1.13.7.5369.
I had problems similar to yours detailed above in this thread. Roll back to 1.13.7.5369 has been trouble free for about 2 months now.
I use mostly android (mobile) and Plex Media Player on Linux for my clients. I honestly donāt do a whole lot of live TV with Plex but definitely do a lot of DVR recordings. I watch them mostly after they are done recording. Upgrading to 1.14 helped me with being unable to tune live tv channels and losing recordings.
I have the problem with an Ethernet tuner, just started recently. I canāt downgrade to 1.13.7.5369 because I canāt figure out what the link would be. I have a 32bit QNAP so the extension would be _x86.qpkg. Can you at least provide us with the link to downgrade Plex so we can use our service till Plex figures out whatās wrong?
Running 1.14.0.5470-9d51fdfaa and Iām still having the same issue. Maybe this bug was re-introduced? Iām using an Ethernet HDHomeRun Dual HDHR3-US. Iād just like to downgrade to 1.13.7.5369 or whatever version works. Too much to ask?
Iām again having the issue now again with 3 WinTV DuoHD tuners across multiple platforms using PMP on Linux and Windows, plus Roku etc.
Iām also running the latest and greatest 5456? Version, and the devices after a whole drop out with āPlayback Errorā something that was sorted out before has now shown itās head again.
We really need the developer to wake up and smell the coffee and sorry to say, stop adding crap features and fix the issues that are blatantly obvious, transcoding and live TV!
