I just added a tuner on my Nvidia Shield and the Plex Tuner DVR works great. Does the guide update automatically or do I need to manually update? I could not find documentation on this.

Usually it is automatic.
Plex recently experienced a provider change from Gracenote to Tivo and you may have to do a full refresh to pick up corrections.

Since July. 
Plex is still experiencing fallout from the change. USA customers have been the least affected. Sports fans and people that sort by genre aren’t happy at the moment.
The guide updates automatically every 4 hours looking for any changes that relate to the next 24-48 hours of program data and also tries to extend by 1 day after day + 13.
This process is under review and there are already some tweaks made that will be released in upcoming version of Plex Media Serveer
There is a need to do manual refreshes of the EPG through the interface available on Server Settings page for Live TV and DVR. This is necessary to pick corrections to the guide data and also extend the data beyond what may be available - for channels where it stops on day 9 or day 10.
I would suggest doing a manual refresh every 2-3 days - or in event of noticing an issue with the data - then do a manual refresh before reporting the problem as that may fix the problem
