I go through the dvr setup and get this -
Looks like everthing should be okay…then I go to this -

Sorry your screen shot post is kinda messing up your post and hard to read…
You should see this right under settings:
Can you check under Settings → Server → DVR and Live TV and see what your tuner shows:
Thanks for helping,after running setup…(multiple times) nothing shows under manage…under settings
This is after “successfully” running the dvr setup…I can get all the way through the dvr setup…
It’s an HDHomerun…Emby can find it…The hdhr windows and android apps also find it…
I do have to enter the ip address manually. It has worked well for 4 months or so…just not anymore…
After a “successful” setup when I go back to live TV and dvr it wants me to setup the hdhr again…
Based on logs it does seem to be having issues picking up the tuner.
Mine does auto detect the HDHR so I do not have to enter the IP manually.
When did it stop working?
did you install any recent software including windows updates around the time it stopped working?
I did have some colleagues looking at this as well and this was spotted in your logs.

So it looks like somehow your epg db got corrupted and is causing Plex problems when loading the epg data which is probably causing the tuner to not appear.
The suggestion at this time is that you stop Plex, navigate to the folder indicated above and delete the db files and others which may be named similar so in my case these are the files I would delete and restart Plex again and try -
