Would like a clip - perhaps a 10 minute clip and steps to reproduce this
Information that would help
- does it happen with sidecar subtitles or subtitles within mkv or both
- is it on local connections or remote wan connections or both
- does it happen when not seeking forward / back or resuming paused playback?
- any specific Plex Web browser ?
- is it happening with both bundled plex web (accessible through http://127.0.0.1:32400/web or http://ip-of-server:32400/web) and hosted plex web (accessible through https://app.plex.tv/desktop ) ?
I have already seen the post that says it is happening even without automatic quality adjustments
For reproducing the problem - i would need all the settings that are in use for Quality / Debug / Player
and please try to get logs captured soon after the loss of the subtitles - and note down the time please as close as possible to when it happened

For providing the clip - if it is an mkv you can use tools such as mkvtoolnix to get the first 10 minutes of the video
