Plex mapping same video file twice, shows up as duplicate

This problem recently started happening a month ago and has been happening more and more frequently. Plex appears to be scanning the same video file twice, where both copies are linked to the same entry, and then show up as duplicates.

Every one of these ^ movies has the same problem.

My Plex Media server movie library spans across two folders:

And I’ve tried emptying trash, rescanning, etc to no avail.
What should I try next?
