Plex Media Server Libraries offline locally

Running Plex Media Server via Truenas Scale Fangtooth 25.04, running Plex v1.2.15.

I can login to Plex web server via server IP (or clicking Web UI button) and everything is there. I cannot access my libraries on any TVs or devices on local network. My NAS and TV connected via ethernet, other devices are local Wi-Fi. I have tried the login/logout/reset cache tricks to no avail. I have fiddled immensely with router and server settings on all three ends (router, Plex, Truenas). Settings are currently as follows:

Plex Local Networks: 10.0.0.0/8, 172.16.0.0/12, 192.168.0.0/16

  • Host Network is NOT checked.
  • Host IP is listed ‘0.0.0.0’ IP Address
  • Server sits on a static IP address assigned by router

Truenas network: 192.168.1.204/24, 2600:1700:4080:cb0:7285:c2ff:fe7d:3e52/64

Router (Humax BGW320-500): 192.168.1.254

  • I have this port forwarded best you can with this router, default 32400.

I have also messed with permissions immensely, I believe I have this correct. Media dataset is fully shared with user1, root, and apps. SMB share is on. I can access the media folder from a mapped network drive on PC and edit folders with no issues. I’ve spent all week on this and am losing my mind.

Feels like a network settings issue. Please assist. Do I need a new router? Can provide logs. Might be worth mentioning it has been up and accessible locally and then just goes unavailable with no intervention from me.

Update: enabled Host Network (checked box) on Truenas Plex app configuration and this seems to have me in business. If it was that easy what was the fix I was missing without that check box?

Also the server is no longer available outside of my network. Port is forwarded via NAT/Gaming. Looks like this:

I think this is the only option with this router as uPnP is not available.
