That snippet from your logs only shows that the server had crashed and Plex attempted to upload information about the crash. There’s a limit to how many crash logs can be automatically uploaded… given you said it’s a recurring issue, it seems you’re already over that threshold, therefore they’re not uploading yet another version of that file.
Plex are usually looking at those crash logs provided automatically on a regular basis.
If you’re looking for help here in the forum, you’ll better provide the full logs (at least the 6 Plex Media Server.log files).
Aside from some issue with your remote access / UPnP setup those logs only let us guessing. There might be an issue with the hardware transcoding. To rule this out you could disable it in the Plex Media Server settings to see if playback works without crashing under those circumstances.
For a more detailed look, please check Settings > [Server Name] > General > Enable Plex Media Server debug logging (but not verbose logging!!), then recreate your crash and upload a new set of logs.

There’s some issue with your remote access setup that seems to cause some networking issues and an indirect connection for remote clients.
Some of the logs show some hiccups with hw transcoding.
But as you said… none of those appear to be fatal.
Ok, thanks. I’m going to keep an eye on it and maybe just rebuild the server at some point. I’m not sure what else to do. I’ll also do some experimenting to see if I can nail it down. So far it seems intermittent and entirely unrelated to the playback of media. I’ve adjusted some firewall rules to allow more incoming connections and will test enabling debug again.
So far, really unimpressed with Plex’s automated crash upload tool or their ability to even log these crashes. Clearly that’s a work in progress on both end if they aren’t catching this and also limit the crash dumps.
What’s also interesting is that this issue has progressively gotten worse over time. It used to only happen once a month (no big deal). Now it seems to be happening every other day (or more). And this is after keeping the software updated which presumably would fix bugs. Heh
