For the fun of it I did a run w/ process monitor running, PMS definitely does and can access that mkv:
I’ve made a stub exe that logs command line and executes ‘orig.exe.save’ and replaced Plex Media Scanner.exe.
For Jett mkv it does not get invoked at all and this is log when kevin mkv is in dir:

So the question I guess is what can cause PMS to skip invoking scanner.exe ?!?!
here is another one to scratch your head… just for the fun of it I’ve created a new user and ran PMS under that users context and it worked

As I was about to tell you Plex won’t find my copy of Star Wars Clone Wars (2003) I noticed the file names were missing the closing parenthesis around the year.
so instead of Star Wars Clone Wars (2003) - S00E01 - Volume 1 they where named Star Wars Clone Wars (2003 - S00E01 - Volume 1. As soon as I fixed that plex found it.
For the OP, it’s interesting that on a new user it works as expeted. I feel that there may be some corruption in the ACLs of these files. Try this:
On the root folder of of your media, in the advanced file permissions window use the change ownership, either select a new principle, can be your self even and check the Replace option
after it processes close out of all the properties windows.
open them again make sure the permission entries look like this at least. you can give yourself full control if you like. and make sure the replace at the bottom is checked before hitting ok
Once you are done with that, tell Plex to rescan to folder.
You may also want to compare your file and folder attributes for the show that won’t scan as well to ones that do.

there are two things that it is usually when you everything else tells you it’s not. DNS and permissions. and when it’s not DNS it’s permissions. It won’t hurt to try resetting the permissions on your folders instead of dismissing it out of hand

if it was such a problem the old scanner wouldn’t work either…
additionally, adding a TV-Shows w/ these file on a e. g. FAT formatted usb stick has the same problems. and w/ FAT you certainly won’t have permission problems of any kind
