Just want to mention that I have the same problem (random episode missing with multiple multi-episodes within a season) and the “random episode” which is missing is different with different orders of addition of multi-episodes.
Naming convention has been respected.
If somebody seriously wants to hunt down this bug, I am happy to produce some more details and logs. I will be unavailable due to traveling during the next couple of days, but I am happily assisting somebody who wants to analyze the cause.
This is a relatively recently introduced bug, so don’t try to guide me through database integrity speculations and plex dance trivia. These are not causing (or fixing) the issue.
Of course, every reasonable developer could just add a couple of multi-episode files to the same season in order to try to replicate.
P.S.: It looks like some kind of indexing problem.
I can recreate the issue.
It seems to depend on which syntax of the two possible is used, so either the first or the second episode number is recognized. And only that single one.
Side remark: season 3 of Recess does only have 16 episodes on TMDB (i.e. 8 double episodes).
You may want to check which episode ordering you are using with your files.

I couldn’t recreate it trying myself but with your info I can try again and that would be Docker (for my test server it is a Debian host and I am using the Linuxserver.io image). Let me know if that would be helpful for you.
@Sparkster36 thanks for mentioning what you see in the logs. Since you got @OttoKerner here, I think we are good now since it looks likes they were able to reproduce.
I do want to mention that I have a show with 52 episodes that are actually 26 individual “double episode” files. It has 3 seasons and they are all good, so based on my observation it does not seem like there actually is a limit for this (not that that means I am correct).
I posted this in another thread - I had the same issue yesterday and today trying to add a single file of episodes S03e01-E02 of Defiance (but exiting multiple episode files were still ok)… My solve for now was to change Defiance to use The Movie Database (Aired) for Episode Ordering. This makes me believe that the fault is in the default library.

All existing shows are fine, new shows added with multi episode files are not scanned correctly.
It’s always the first of the episodes missing in a multi episode file, when naming the file like this S01E01-E04. Episode 1 will not show in library, E02, E03, E04 are ok.
Same problem for me.
like @truthgalaxy, all existing shows are fine, and this problem occurs only for new shows or renaming existing shows.
i’m using latest version of PMS (1.40.3.8555) with official docker repo, on Ubuntu 22.04
I’m having the same issue on MacOS server. It will recognize the second episode, but not the first, so S01E01-E02 only shows E2, S02E05-E06 only shows E6, etc…
Just to let you know this looks to be resolved in the latest beta per the notes.
The latest episode of Marvel’s Spidey and His Amazing friends I added about 2 weeks ago exhibited this bug where it only showed episode 26 and episode 25 was missing.
I just updated to the beta today and ultimately I had to Plex Dance the episode. After I did that and put the episode back and refreshed metadata for the series, both episodes are now appearing. Note that I did try just refreshing the metadata first and the missing episode did not appear, so if you have multiple episodes impacted by this, probably easier to Plex Dance either the season or series (depending on what level the issue is happening on).
