Twice this month the content in the program guide disappears and it is empty. Only a manual refresh brings it back. As a result all DVR recordings fail.
Please advise.

Me too, running on a Mac. I cannot bring my program guide back and DVR is now not useable. It was working fine one day, completely gone the next. I have tried rescanning the channels (it finds them fine) and refreshing the guide multiple times. No luck. I will not be renewing my subscription if this can’t be resolved… Plex support, please help!
I had this issue 2 days ago (I use Apple TV, with the server running on a Mac mini). Woke up to find that the Program Guide was completely empty. I tried reloading multiple times & nothing. Then I tried refreshing the guide, and it stopped a few times around the 50% mark. I updated the software, tried refreshing, & nothing. I signed out via ATV & back in, and did the same on the Plex server. I then was prompted to relink my account. I also got a message to secure my server which I did. I refreshed the guide again, and everything is finally back & working again. What a PITA, though.
I might as well chime in here. Upgraded to Version 1.12.1.4885 on macOS High Sierra 10.13.3, deleted the tuner, added it back, tried to refresh the guide. Nothing. Not an auspicious start for a new Plex Pass subscriber.
Update 2018-03-29: Tried to delete and add the tuner back again. Three times it hung at 0% at “We’re finding the best program guide for your channels and location. This should only take a few moments.” Once it showed the list of providers. I selected “Local Over the Air Broadcast” and it had an error contacting the provider. Tried again, and it showed a list of channels that I had to remap (it should be able to figure out the one-to-one mapping). It started to download, showed 1%, stopped a few seconds later claiming setup was complete aaaand … the program guide was empty.
