Playback on the PS4 app is sometimes a small postage stamp in the middle of the screen. The same videos play fine on all other apps (IOS, Android, Sony TV).
I can’t see any settings in the app to fix this, and the TV is set for “Screen Fit”.
I too have this issue, i’ve read about widescreen; anamorphic widescreen & 4:3 ratios but no fixes. This is what I get on the PS4. I have the latest version of the server and PS4 app both down loaded yesterday. I used makeMKV to rip. Plex looks awesome but if i can’t solve this its back to the drawing board for a solution 
The rest of plex fits the whole screen perfectly, counters timers etc are all where they should be, it’s just the video that the wrong size.
I’m really just looking for a way to get this right, does anyone know how to solve the issue?
I’m also experiencing the same problem & I can’t find anything in the options to fix it. In saying that when I’m using plex on either my mac or through my Samsung it stretches to the full screen perfectly.
Logged in to say I’m having the same issue, Xbox One/360, iPads etc are fine.

I have the same issue with either the PS3 or PS4 Plex App. 480p (16:9) MKVs (DVDs encoded via MakeMKV 1.9.9) play and scale correctly on my Samsung Smart TV or my PC or my iPad or on my PS3 or PS4 through the DLNA media player (without vertical black bars on the sides).
