Remote server only visible via plex.tv, not local web client

I have a server running on my Synology DS218+ which is only visible via http://app.plex.tv/desktop but not via the local web client installed through my Mac Plex server. The “Remote Access” tab says it’s “Fully Accessible outside your network”. I’m signed in with the same account on all 3 locations, although the issues seems to have started after a recent password change - after which I had to sign in again in all 3 locations and claim my server again.
What else could I check to fix this?

I have a PlexAmp client on the same Mac that can access the server, similarly an Android with PlexAmp and Plex clients successfully connecting to the server on the same WiFi as the Mac is, as well as on cellular.

Worth noting the locally installed “newest available” web client version (4.55.1) is older than the app.plex.tv version of 4.56.4.

This sounds simple but I fail at step zero by not being on the same subnet (or same physical location) as the NAS. Although right now I can access the NAS Plex using app.plex.tv/desktop or the externally exposed IP/port I’m concerned signing out would kill this option as well until my next physical access. Are there any less intrusive debug or recovery steps I could try?
