Scan finds only some of new files added

I’ve recently added a TV show to my hard drive. When I run the Library Scan it only picks up 5 of the 14 episodes. For another show Plex is only finding the first episode of each season. All are MKV files, in 1 folder, same naming structure. The files play fine directly from my drive, but Plex just won’t register them.

Any tips?

Mind sharing the folder structure / episode naming?
Edit: do the other episodes show as duplicates of others or not at all?
