Seemingly random failed recordings with error: Could not move recorded media to destination

I was able to recreate the issue in the middle of the day when it was more convenient to download the logs immediately after the problem occurred. Please see attached log files. The recording that generated the error in this case was Between 2021-06-16 16:00:00 and 2021-06-16 16:30:00 on channel 7.2: ‘Adam-12 - E18 - Man Between’

As you can see in the screenshot below it listed it as being recorded twice while the recording was in progress. The second screenshot shows the “Could not move recorded media to destination” error. The third screenshot shows the destination folder contents. The file for the failed recording “Adam-12 (1968) - S03E18 - Man Between.ts” is in the destination directory along with two other files that were left behind:

There were two other successful recordings happening for two other shows during this time frame.

Plex DVR is recording show twice

I have noticed that since I added a second SiliconDust dual tuner to Plex my DVR is now recording the same show twice at the same time. It just uses both tuners all the time, defeating the purpose of having the second device. Seeing as this is for OTA I am thinkingt that I will split up the enabled channels on each and this will resolve that particular issues. But it is annoying. See the attachment for an example. Plex saving 2 copies of the same TV Show

I have been using plex DVR almost since it has been released and all of a sudden on one particular show it saves 2 copies of the recording. In the pictures that I am attaching it will show that only 2 tuners are being used with 3 shows being recorded 1 show twice and no post processing script. Plex DVR recording show multiple times

Server Version#:3.95.2 I’m assuming I need to rebuild the DVR schedule so these duplicates don’t show up? As the picture indicates, I’m recording Dateline twice. I’m assuming the database needs to be rebuilt. How do I accomplish that? [plex]
