Have you narrowed this down to MKV files only, as the topic implies? Do you have any other kind of media that ISN’T MKV to try this with? Maybe use Handbrake to convert a file to MP4 and see what happens.
The other thing you can do is check the logs for your server. It may help to enable the network logging from the Plex App on the Xbox One before trying to play a video that is not working.
After playing about 30 video files (.mkv and mp4’s), mp4’s are definitely not affected. Most .mkv’s that were added to my library post last month work fine.
The issue is with newer .mkv files I’ve added in the last week. I converted one of the files I was having an issue with from mkv to mp4, and it played without issue.
I’ve attached the server log, but I haven’t the faintest what I should be looking for.
During your playback in that log, was the trouble you were having while trying to watch the show Alone?
Since the log covers a 2 hour period, I just need to try and narrow it down a little. Ideally, the best thing to do is restart Plex Media server and then attempt to play a problem episode and the restart the server again after getting the error/a minute of it trying to play. This will cut the log down considerably and be more focused on the problem area. (If you do this the log you will be looking for would be ‘Plex Media Server.log.1’ as the log will rollover after the last server restart)
Yes, that was one of the series. The S2E1 of Strain also wasn’t playing. Interesting tidbit - after server restart, I tried to play S1E1 of Banshee (which worked a couple of hours earlier, this time I got the error).

Awesome, yeah looking the log you have the 10054 which means the Xbox closed the connection to your server. There was also a video error code 3 which I have no idea what that means. But will definitely forwarding the information on to the team so they can work their magic.
Alright, so looking a second time the problem I am seeing (and what I highlighted to the devs) is what actually looks like invalid request ranges for playback. What I mean is if you search for ‘range request’ you will see the file bytes and in instances with failed playback you will see the starting byte is close to the max file size which is the end of the video, a valid one should start at 0 (at least for video starting from the beginning).
I believe this is what is leading to the Xbox forcing the connection closed as it is getting to the end of the file and thinking it’s done. But this is where the Dev’s will need to work their magic to try and find out what is the actual cause and what is just a symptom.
Well I transferred over a couple of new titles to the library last night… Both played without issue, in the identical format as the ones that are having issues.
While trying to reproduce and get logs for the developers last night, I was also noticing that maybe 2 out of 10 times the video would play fine. But hopefully, the new library keeps em working 100% for you.
I’m not sure what to look for in the logs so it would be great if someone here can have a look and let me know if there are any clues there as to what may be happening.
Just like the above user, I have converted the MKV files to MP4 and they play just fine. The glitch manifests as green lines running horizontal across the screen especially in segments where there is lots of fast moving video. Is it possible the xbox one player has issues with fps above a certain number? Some MKV files such as Mall cop 2 I watched last night play without any issues at all. Others like American Sniper and Jupiter Ascending are full of this glitch.
I’m seeing the same exact issue down to the 10054 error in the log. This affects about 5 videos out of a season Of 11, while the others play just fine. I tested on a few other devices and playback works just fine. Disabling direct play on the Xbox one gets the problem files to play correctly. However, in the now playing section of my Plex server it still lists it as direct play. Let me know if you guys need any additional logs? Just wanted to share my experience

This is a bug in the current version of the server. If you want to be 100% sure you can check your Plex Media Server.log and when you start a video you will see a call to the transcoder and should see a line mentioning that direct play is disabled.
