Same exact problem here, running the latest PMS on Windows 10. Fairly recent issues, too… my folders haven’t changed in years… suddenly, it looks very similar to what you’re describing.

You had a mismatch at the Artist level. To clear that up, you need to remove that artist completely from the database. That requires Plex Dancing all albums of that artist and also all other albums which are involved in the mixup somehow.

I thought my settings “Prefer local metadata” meant my folder structure meant nothing.

Performing that “Plex Dance” I loose my playlists I have made.
