If your file is an avi; it shouldn’t have any metadata hidden to screw it up as @OttoKerner says. It might just be a hiccup in the TVDB servers. Sometimes if you go away and try it later it works perfectly.
I just tried it as test file and it came right through; the only difference is that I put the title along with the season/episode number and that shouldn’t make much difference.
I’d say, add the episode name (just in case); do the Plex Dance and try it again. It should work without the episode name in the file; but it did work for sure with the name. . .
