I have this issue off and on with all different file types when streaming to a Roku 2 running PMS on a Win XP machine. I'll start a movie, fast forward anywhere from 5-45 minutes into the movie and after I press play the load bar will hang around 1/3 for a couple minutes before giving me the error message "Video unavailable." I have tried the same movie files from a network drive, an ext drive plugged direct to my PC and the file on the hard disk in my PC with the same results. Some files will play for a couple minutes and then just quit and give me the same error without fast forwarding. It has been working fine forever with the same setup, this is a recent development. (Pretty sure none of the files I play are MKV either, I know that tends to be a troublesome format.)
Bumping this topic as I've been experiencing the same thing. I have confirmed that I have the most recent version of the Plex Media Server, and have also removed & re-added the Plex channel on my Roku to verify that I have the most recent version of it. Can one of the Plex developers please respond?
Also have this issue tried both Plex and PlexTest channels

Seems it's related to reference frames... quick search on the forums led me to the max of 8-9 and the videos with problems are 10+ so that's that for me
I see that you posted here that you had the same issue. I came here looking for a solution myself, but I noticed that you responded to post made in 2013. Have you found a solution? It seems to be (at least for me) a problem with the transcoder.
