What is it causing this and how a can I fix? Finally how can I prevent this from happening again?

How do you have the following configured on your server?
Settings (server name) -> Library -> Empty trash automatically after every scan
If it is enabled and your media becomes unavailable for some reason and a scan is performed, the missing media will be permanently deleted from your library. If this occurs, you’ll have to rescan your library.
Media may become unavailable for many reasons. A common one is that it is stored external to the server, e.g. a network share and it becomes temporarily disabled. If you have any externally stored media on your server it’s best to leave the setting above disabled.
