In the latest plex server, transcoding with hardware acceleration causes server to crash. This did not occur in the prior version of Plex. When hardware acceleration disabled, transcoding succeeds.
You can find prior versions of Plex in your %PLEXLIBRARYPATH%\Plex Media Server\Updates
You’ll need to uninstall newer versions before you can install older ones.
I successfully uninstalled the latest version of Plex and downgraded to one from October. That fixed nothing. I then uninstalled that version and reverted back to an August version. That also, did not help. With HW transcoding on (i3 9100 using iGPU) my Plex server crashes immediately. Turned off, it works great.
I do remember rebooting my Plex Server running Win10 about a week ago, and applying updates and restarting. That’s when I think everything went to ■■■■. I direct play in my house, so I didn’t notice immediately, but when a family member a few days later complained that my server wasn’t working, that’s when I went to work. I think it’s a Windows 10 recent update causing the issues.
Yeah… I tried a few more versions and had the same result. Like you, I rebooted into a big windows update. Direct play in my house concealed the problem. The Intel GPU acceleration began failing for family outside my house. I also updated to the latest Intel GPU driver.
My transcoding tools all use the GPU with no issues, but Plex crashes and exits. I’ll look into what the Plex team would need to diagnose be the issue.

Have tried the whole day, and realized that it may not be my “wrong setting” fault.
Plex 1.18.2.2058 works fine for x264, but immediately crash after click any HEVC file. By the way, HW transcoding doesn’t work for x264. My cpu is i5-8250u, it works fine before.
Fire TV clients are crashing the server.
Intel Drivers newer than X.7000 are crashing on 8xxx,9xxx
