In TV DVR / Browse/ Programme Guide / News in Grid View, all Icon images have gone/ Still have them for all movies and a few shows but now a lot of arrows in boxes. This was working fine until last update. In the UK using ASUSTOR NAS 6404T and TV Butler. System otherwise working fine. Source data is I believe Gracenote so not sure if its a Gracenote or Plex issue. Also when a channel is off air, used to get a testcard image - very sensible, now just get this arrow in a box. Any ideas?
Having the same problem here. I tried removing and reconfiguring the tuner to no avail 
Thanks for letting me know, think its a gracenote issue or plex linking to gracenote issue. I reconfigured tuner and epg with no effect
same issue as well. I tried troubleshooting, so I deleted the dvr and added it back and all channels had icon images. I then refreshed the guide, and it went back to having no images for some programs. so who knows what’s going on. 
I have worked out what the issue is but do not know how to get PLEX to fix it. All the icons eg all the BBC news icons are still on the TVDB.com website that PLEX uses TV/DVR metadata for. However despite the metadata agent being ticked in the PLEX server settings, the link is obviously broken / not pulling in the metadata art anymore. If anyone knows how to alert PLEX to this it would be helpful.

Yes that’s what I get, on Programme Guide News they are all arrows now. And yes it is getting worse each day with less image art just arrows.
I’m having the same issue as well. It’s on a Windows 10 server and shieldtv server. Same channels on both servers.
Problem solved after much research. Caused by issues with the EPG setting at “Freeview” even though watching Freeview TV. If one resets the TV DVR delete and retune then set the EPG to “Over The Air” setting it all comes good. This should work for all PLEX DVRs tuned into UK Freeview Over The Air Transmissions.

And what about the stations in Germany? There is not “Freeview” and the problem persists!

It has to be a problem with Plex. I went into my dvr settings and unchecked some channels so that they wouldn’t be updated in the guide. When I unchecked enough that only 50 channels were being displayed, all of the thumbnails showed up. When I added 1 more to make it 51, some of the thumbnails disappeared. It’s a weird bug for sure.

Yes it looks like a much bigger problem. Maybe what I found just masks the issue peculiarly for the UK Freeview transmissions rather than any other countries as previously the Freeview EPG worked just fine for me.
I have decided there is definitely a serious PLEX EPG alignment issue. Problem came back after two days!. Both a TV Butler and my HD Homerun DUO failed to correctly align the EPG with many missing channels despite picking up all the correct channels from the local TV Transmitter. To compare I used the HD Homerun with the Channels app and the device tuned perfectly with the correct EPG. I think the PLEX EPG code link needs a complete rewrite- its now far too flaky in my view.
Selecting “over the air” fixed everything for me with my EPG. Thank you very much!
I did this too and it lasted a couple of days before reverting back to missing icons, especially news programs and loss of the sign off test card!!! Very odd.
This is fixed now in latest plex server release.
