Upgraded Hardware, now I have no server

I upgraded my motherboard and OS Hard Drive. I made a fresh install of Plex Media Server. Now, plex web app sees 2 servers of the same name, one with no media added & one not accessible.

I’ve been trying to copy my old PMS metadata files to the new install, but nothing is working. I REALLY don’t want to rebuild my libraries, collections, etc… in Plex. Is there an easy way to import/migrate my old plex server data to the new Plex install?

OS - Ubuntu 20.04.3
All media (except music) is stored on 4 TB hard drives (no UUID changes). My system is configured with a static local IP (same IP as before the upgrades)
