This happens sometimes if a picture is not available on TMDB and Plex “crawls further” to fetch a picture. Take a look here: https://watch.plex.tv/en-GB/show/oggy-and-the-cockroaches
Look at season 2 episode 3. The image is incorrect (trust me on this). The episode has no picture on TMDB. The episode is correctly linked on TMDB to the TVDB episode which has the correct picture.

It’s coming from Gracenote. Looks like it’s matched to the wrong episode (“Love and Kisses”). I’ve asked somebody to look into why this isn’t matching to the correct one.
That explains it I guess.
Note though that most of the episode pictures for season 2 is wrong. That one was just an example.
I’m using TMDB as the sorting base. TVDB has a different order. But the episodes are still linked up to the correct ones via TVDB IDs input into each episode on TMDB. This show is a complicated one, with varying opinions on ordering etc, which is why it’s so important to use the external IDs on TMDB. This is perfect example of that.
I do wonder if you where to add the correct episode thumbnails to tmdb would it help, especially if you use tmdb episode ordering ? It might force the plex metadata cache to update with the thumbnails and ditch the wrong ones from Gracenote.
Although, I tried to do something similar with John Hunter Bell and that’s still an issue and remains an annoyance to my wife!
In my experience, Plex uses TMDB as a first priority source for episode thumbnails, so yes that would work I assume.
